p-leader based classification of first stage intrapartum fetal HRV

چکیده
Interpretation and analysis of intrapartum fetal heart rate, enabling early detection of fetal acidosis, remains a challenging signal processing task. Recently, a variant of the wavelet-based multifractal analysis, based on p-exponents and p-leaders, which provides a rich framework for data regularity analysis, has been proposed. The present contribution aims at studying the benefits of using the p-leader multifractal formalism for discrimination of intrapartum fetal heart rate. First, a dependence on p of the multifractal properties of data is evidenced and interpreted. Second, classification between healthy subjects and fetuses suffering from acidosis is shown to have satisfactory performance that increases when p is decreased. Keywords— wavelets, multifractal analysis, p-leaders, intrapartum, fetal heart rate variability, acidosis detection.

Intrapartum heart rate ambiguity: a comparison of cardiotocogram and abdominal fetal electrocardiogram with maternal electrocardiogram.
OBJECTIVE/AIMS To investigate the presence of signal ambiguity of intrapartum fetal heart rate (FHR) monitoring during delivery by comparing simultaneous cardiotocogram (CTG), abdominal fetal electrocardiogram (ECG) with continuous maternal ECG. METHODS A total of 144 simultaneous CTG (Corometrics 250 series), abdominal fetal ECG (Monica -AN24™) and maternal ECG (Monica AN24™) recordings were...
متن کاملTHE EFFECT OF INTRAPARTUM EPIDURAL ANALGESIA ON NULLIPAROUS LABOR
Our purpose was to determine the effect of epidural analgesia on nulliparous labor and delivery. Nonnal term nulliparous women in spontaneous labor were divided into two groups in a quazi-experimental study, 100 in each group. The first group received epidural analgesia and the second had no analgesia at all. In the first group, an epidural catheter was placed in 3-4 cm cervical dilatation ...
متن کاملAutomatic evaluation of intrapartum fetal heart rate recordings: a comprehensive analysis of useful features.
Cardiotocography is the monitoring of fetal heart rate (FHR) and uterine contractions (TOCO), used routinely since the 1960s by obstetricians to detect fetal hypoxia. The evaluation of the FHR in clinical settings is based on an evaluation of macroscopic morphological features and so far has managed to avoid adopting any achievements from the HRV research field. In this work, most of the featur...
متن کاملDetection rate of fetal distress using contraction-dependent fetal heart rate variability analysis.
OBJECTIVE Monitoring of the fetal condition during labor is currently performed by cardiotocograpy (CTG). Despite the use of CTG in clinical practice, CTG interpretation suffers from a high inter- and intra-observer variability and a low specificity. In addition to CTG, analysis of fetal heart rate variability (HRV) has been shown to provide information on fetal distress. However, fetal HRV can...
متن کاملRole of Intrapartum Ultrasound in Modern Obstetrics
Intrapartum ultrasound has become more popular over the last decade. It provides a more objective assessment of fetal head position and station than traditional digital vaginal examination, which is now known to be inaccurate. Correct identification of fetal head position is particularly useful before instrumental delivery, although it should not be used in the first stage of labour to predict ...
